- Owner’s name, address, and phone;
- Renter’s name, address, phone;
- Current date;
- List of equipment that is rented with the name, description, and serial number;
- Rental Rate;
- Term of the rent;
- Signature of the renter.
Both renter and owner have to read and agree with the terms. In case if there are more items to rent, you can add extra fields. You may also agree on the compensation in case some of the rented items will be lost or damaged.
