    What is form I-693? To protect the health of the United States citizens, non-immigrant and immigrant foreigners must first prove that they are not carriers of dangerous diseases. Therefore, USCIS requires the visitors to submit Form I-693, in which the results of medical examinations are provided. These exams are needed to prove whether the applicant is eligible to enter the United States or not. What I need the I-693 for? Form I-693 is used as evidence that the applicant is not bringing anything into the country that could threaten the population. However, this medical examination should be provided by one of the doctors: If you apply from the US: Contact a civilian surgeon who is a doctor appointed by USCIS; Applying a statement from Abroad: Find a doctor who is an authorized U.S. Department of Health.     What is an Affidavit of Support Form I-864 [year]? Affidavit of Support or I-864 is required by most family immigrants and some prospective employment-based immigrants to demonstrate that they have adequate financial support and are unlikely to become a public charge. The sponsor fills out the I-864 on behalf of the future immigrant. What I need the I-864 for? This is a contract between the sponsor and the US government. If you are applying for an immigrant visa as the child of a U.S. citizen or LPR, you will be required to file the I-864 unless you are applying for a K-3 or K-4 nonimmigrant visa.     What Is a Supplemental Document I-9? Form I-9 serves to documentarily prove the identity and employment authorization of workers hired in the United States. The first form supplement of this document is Preparer and/or Translator Certification, which a specialist of this profession has to complete to attest that they have assisted in filling out Section 1 of this form and that the information provided in the document is correct. What Do I Need the Employment Supplementary Form I-9 For? Supplement sheets for Form I-9 are required in the following cases: If you were rehired, and more than three years passed after you completed Form I-9; If you asked a preparer or a translator to help you fill out the First Section of Form I-9, you need to provide Section 1 Preparer and/or Translator Certification to ensure that all the data provided in the document is accurate.     What is form I-134? Affidavit of Support or Form I-134 is a legal document used to show the US government that a visa applicant is sponsored by someone who is already in the United States.
