Step 5: Enter your establishment number, if applicable.

Step 7: Fill in the employer's name.
Step 8: Enter the employer's address and ZIP code.
Step 9: Specify if another EIN was used.
Step 10: These include wages, tips, various tax withholdings, and other compensation details.
Step 11: Fill in state and local wage and tax information.
Step 12: Provide the employer's contact person, phone number, fax number, and email address.
Step 13: Sign and date the form, providing the title of the signing officer.
