What is a Transaction dispute form? 

The Transaction dispute form was designed to allow the individuals to claim their disagreement with the further charges from a merchant that has billed their card. Anybody can fill it. 

What I need a Transaction dispute form for? 

You can submit the Transaction dispute form if you:

  • Are dissatisfied with the quality of goods;
  • Want to get a full refund;
  • Bought the goods or services by non-cash payment. 

In the form, you can provide detailed information about your disputed money transactions. 

Filling out a Transaction dispute form

The form has three pages. Submitting this document, you also need to include transaction receipts and correspondence with the merchant. There are five sections:

  • Section A contains information about cardholder;
  • In Section B, you need to provide the information about disputed transactions;
  • In Section C, specify the dispute reason. There are some examples you can select from;
  • Section D contains a claim number field;
  • In Section E, the cardholder needs to sign the document and put the date.
