What is a Transaction dispute form?
The Transaction dispute form was designed to allow the individuals to claim their disagreement with the further charges from a merchant that has billed their card. Anybody can fill it.

Filling out a Transaction dispute form
The form has three pages. Submitting this document, you also need to include transaction receipts and correspondence with the merchant. There are five sections:

- In Section C, specify the dispute reason. There are some examples you can select from;
- Section D contains a claim number field;
- In Section E, the cardholder needs to sign the document and put the date.
