501c3 application form is the document that needs to be submitted by an applicant in order to get the grant. It refers only to charitable contributions.

The form is very short, there’s only one page. First of all, you need to choose the category of grant type. There are seven categories you can choose from: Children & Youth, Education, Environmental, Health & Human Services, Civic & Community, Arts & Culture, Minority. After that, provide information about:
- Purpose of the grant;
- Funding sources;
- Amounts already secured;
- Methods of measuring the effectiveness of the program;
- The area of activities;
- Business relationships the organization has with Aflac;
- Aflac employees;
- Board directors.
Besides this, you need to attach the documents listed in the form on the bottom of the page.
