  5. Fill in your birth state or country if you were born outside the United States. Next, select your ethnicity from the available options. In the race field, check all that apply. 
  6. Indicate your primary spoken language and any other languages you commonly use at home. 
  7. If you're currently under long-term suspension or expulsion, indicate 'Yes.' If not, mark 'No.' 
  8. Indicate whether you've previously attended a Shawnee mission school by checking the correct option. 
  9. Clearly state whether you have an I.E.P (Individualized Education Program) or 504 plan by checking either 'Yes' or 'No.' 
  10. In the 'Primary Residence Contact Information' section, provide your home address (street, city, state, and zip). 
  11. Fill out the names (Last, First, Middle) and relationship to the student for Guardian 1. Provide the primary phone number, secondary phone number, additional phone number, email, and employer details for Guardian 1. 
  12. Repeat the same process for Guardian 2. 
  13. Under the 'All Children Residing at Residence' section, provide the last name, first name, birth date, and school for each child residing at the same residence. 
  14. In the 'Emergency Contact Information' part, fill out the last and first names, title, and relationship to the student, and provide contact information (primary, secondary, and additional phone numbers) for two emergency contacts. 
  15. Lastly, endorse the online student enrollment form by adding your signature and current date.
