What Is NFA Responsible Person Questionnaire?

NFA Responsible Person Questionnaire is used to report the intention to transfer, make, or acquire a firearm. This form should be filled out as a duplicate for CLEO. It will be attached to other documents including the ATF Responsible Person Form.

What Do I Need NFA Responsible Person Questionnaire For?

• To report U.S. Department of Justice that a person is buying, transferring, or providing a firearm; 
• To make a questionnaire that confirms the person’s right to do so;
• To attach this form to other required documents to provide additional information about the person and firearm type.

How to Fill Out NFA Responsible Person Questionnaire?

The fillable questionnaire has eleven questions that sometimes contain several lines. 

1. First, you should indicate the PDF form you will attach to this blank (ATF 1,4, or 5).
2. Then you should include your personal information. The data provided in NFA should be written in the same format as in ATF.
3. The next questions refer to the type of firearm. If you don’t understand some definitions, you should go to page 3 to find the list of all definitions in the form.
4. Section 7 requires you to confirm that you will provide a copy of this form to CLEO. It also contains information about the agency. 
5. Most other questions have “yes” or “no” answers. At the end of the printable blank, you have to sign it and put a date.
