Maine MVT-5: Affidavit of Repossession
What is Maine Form MVT-5?
Form MVT-5 is the Affidavit of Repossession of the Maine Bureau of Motor Vehicles, Division of Title Services. The form says it is for use by lending institutions when a vehicle is repossessed.
What the form says about completing it
The form says it should be completed and signed by an authorized representative of the lending institution, and that it must be notarized. It also says that the lien must be released on the face of the title, and that the form cannot be used to transfer ownership: ownership is transferred on the reverse of the title.
