What is SSA 89 Form?

SSA-89 Form PDF is a one-page authorization you can issue to SSA to disclose your SSN to third parties. As a rule, in situations where confirmation that the SSN really belongs to you is required, the third party will ask you for this permission. So you should not fill out the form unless asked to do so.

What do I need the SSA Form 89 for?

• You must provide this document to SSA so that it can disclose your sensitive data, such as SSN, in some financial situations.
• This permission might be required if you take out a loan or a mortgage. The provider of such services wants to verify that the SSN you provide is correct and doesn't belong to someone else.
• Do not give permission to access the SSN to third parties (lenders or credit check agencies) if you do not take out a loan to avoid becoming a victim of scammers.

How to Fill Out Form SSA 89?

1. Type in your full name, date of birth, and SSN.
2. Select the reason for disclosure that best describes your situation from the list of options. If there is none, write your own version in the last paragraph.
3. Enter the name and address of the company to which SSA can disclose your number. Also, you can specify the name of a specific person/worker (if you know them) who might send a request.
4. Specify the period during which this permission is valid. If you do not specify it, the authorization will be valid for 90 days from the issue date. Sign and date the document.
5. If the SSA 89 Form blank was filled out by the official representative of the SSN owner, they must enter their data in the last paragraph.
