  11. In the "Statement of Compensation Received by Caregiver" section, fill out all applicable fields by clicking on each text field and typing in the required information.
  12. If you cannot have the caregiver complete the PIT-CG form, fill out Sections I, II, and III by clicking on each text field and typing in the required information.
