What is a DS-86 form?

Form DS-86 is used for filing a claim that you, as a US citizen, did not receive your passport, which was issued and sent by the appropriate service. It is in your best interest to fill in this blank, as it will help eliminate the possibility of your identity being stolen and used in dishonest business. You should include a copy of your ID with it.

What do I need the Form DS-86 for?

  • By law, you must receive your passport within 10 days of its issuance. If, for any reason, you have not done so, you should fill out this form online.
  • Before using fillable Form DS-86, you should check the date of issue of your passport on the Bureau of Consular Affairs website or contact the NPIC by phone. Unlike tax forms, this document is not used by the IRS.
  • You have 90 days to submit this document for free. If you do not meet the deadline, you will have to re-apply for a passport and pay the full cost.

How to fill out Form DS-86?

After you have verified that your passport has been issued, but you have not received it, download Form DS-86 to your device or fill out a PDF document on the website. Read the information at the top of the first page and on the second sheet.

  • On the first page, indicate your name, contact information, and also tell how exactly you tried to get your passport. You or your representative should sign this document if you are under 16 years of age.
  • The block at the bottom of the first page is filled by the service you are contacting.
  • For now, you cannot print and send the completed form by mail. You should first contact NPIC by phone or email. Their representative will tell you about your next steps.
