What is Form DS-11?

Form DS-11 is the U.S. Department of State's Application for a U.S. Passport.

First-time applicants use it to apply for a U.S. passport book, a passport card, or both.

If none of its listed eligibility conditions applies, applicants may be eligible to use DS-82 or DS-5504, depending on their circumstances.

Completing and submitting it

Print legibly using black ink only, following the State Department instructions.

At the appointment:

• Present the completed form and attachments to the authorized agent administering the oath.
• Sign only when requested, with the agent witnessing.

Submission is in person at an acceptance facility or a U.S. embassy, consulate, or consular agency (if abroad).
