Form 8821-A (IRS Disclosure Authorization for Victims of Identity Theft) is a federal tax form used for allowing the Internal Revenue Service to disclose your tax return data and related information to a local or state enforcement organization in case you are requesting an identity theft investigation. There’s no need to attach your return to this form as the IRS will identify it by your SSN. This form usually goes after form 14039 and 14039-B (for business).
