What is Form 649F?

The Medical Examination Report Form 649 F is a four-page overview of the driver’s health status. It is used when passing medical examinations to obtain permission for commercial driving. Some of the information is filled in by drivers, and some by medical examiners. The commission decides on the person’s qualification based on the data entered.

What do I need the 649 F Form for?

• The FMCSA requires all commercial drivers to undergo regular medical examinations to obtain a license to practice.
• If you want to start working in this area or plan to continue, you must pass a medical examination and fill out this form.
• If you knowingly provide false information, you might be disqualified.

How to Fill Out the 649 F Medical Form?

1. The driver must complete the first two sections. Enter your name, date of birth, age, SSN, exact address, driver’s license number, etc. Answer a few questions about your health condition. Put your signature to confirm the correctness of the information.
2. The examiners complete the following sections. They should analyze the driver’s answers in the previous parts and give their comments.
3. Next, they should examine vision, hearing, pulse and blood pressure, and the general condition of all body systems and enter the data and findings in the appropriate tables.
4. Based on the tests, conclusions about the driver’s compliance with the standards are drawn.
5. The certificate on the last page is filled in if the driver is found fit to drive. Both the examiner and the driver must put in their signatures.
