Don’t file this form If you haven’t applied for EIN (Employer Identification Number) yet. Use Form 14039 (Identity Theft Affidavit) instead and include your SSN (Social Security Number), ITIN (Individual Taxpayer ID), or ATIN (Adoption Taxpayer ID). 

3. How to fill out Form 14039-B?
