How to Fill Out IRS Form 11C
When it comes to filling out form 11 C, it's all about understanding the different sections and what they require from you.
- Begin with entering the start and end dates of your return period where indicated at the top of the form.
- In the next section, provide your name and employer identification number.
- Fill out the box asking for your number, street, and room or suite number. This is your current street address, including any room or suite numbers if available.
- Enter the city or town, state or province, country, and ZIP or foreign postal code of your current address.
- Provide your daytime telephone number in the allocated box.
- Choose one of the three options depending on your return status: First return, Renewal return, Supplemental registration.
- If your business address is different from your home address or the address entered previously, fill in the 'Business address' section.
- In the section labelled 'Alias, style, or trade name', enter any alternative names you may use professionally.
- Under the section 'Occupational Tax', enter the date (day and month) you’ll start accepting wagers during the tax period.
- Calculate your tax sum and enter it in the appropriate box.
- Select either 'Principal' or 'Agent accepting wagers for another' according to your tax status.
- If you're filling out the form as a principal, provide the necessary information in the 'Additional Information (for principal only)' section.
- If you're an agent who accepts wagers on behalf of others, fill out the 'Additional Information (for agents accepting wagers on behalf of others)' form.
- In the 'Supplemental Registration' section, explain why you’re filing a supplemental registration and include the date of the event that prompted this additional registration.
- At the bottom of the form, there's a section for your signature, date, and title.
- If a paid preparer completes the form for you, they will fill in their information in the 'Paid Preparer Use Only' section.
