  8. In the 'Invoice #' field, enter the unique invoice number for the equipment rental.  
  9. In the 'Date' field, choose or type the date the invoice is being prepared.
  10. Next, you'll see the 'Bill to:' section; here, input the Name of the client who is being billed for the equipment rental. 
  11. Also, fill in the Street Address, City, State and Zip Code connected to the client's billing. 
  12. Now, in the 'Ship to:' section, enter the Name where the rented equipment will be shipped. 
  13. Provide the specifics of the shipping address - the Street Address, City, State, and Zip Code. 
  14. In the table provided, write a brief Description of the Equipment to be rented. 
  15. Specify the Quantity of the equipment being rented. 
  16. Then note the cost per unit, i.e., '$/Unit'.
  17. The 'Amount $' will be a product of the Quantity and the $/Unit, make sure to fill this in correctly. 
  18. Sub-total will consist of the sum of all rental costs before discounts or additions. 
  19. In the next field, if any Discounts are applied to the total cost, enter it.
  20. The 'shipping' field is for any Shipping costs associated with the rented equipment.
  21. Then, determine the associated tax, if any, and record it. 
  22. The 'Total' will be the grand total after considering the above fields - a summation of subtotal, discount, shipping, and tax.
  23. In the 'Comments or Special Instructions' field, state any crucial details or guidelines.
  24. Specify the 'Payment Due' field; this will be the date when the payment is expected to be made by the customer.
