What is Form DS-4151?
Form DS-4151 is a U.S. Department of State form with the printed title "Time and Attendance". The Department's electronic forms catalogue lists it with the description "Time & Attendance Sheet". The file published there is marked 12-2025.
What does the form record?
The form covers one pay period. Its grid has columns for the date, the start time and the end time, and a line for period totals. Under the grid the employee certifies that the regular hours, the leave taken and the premium hours worked are accurate. Two more blocks follow: "Supervisor Action and Certification" and "Time Keeper Certification".
