What is Prop 003 Form?

The Form Prop-003 or Construction Proposal Template was made for contractors who need to notify and receive the official agreement of the owner of the project to use extra procedures or specific additional materials that were not previously discussed. The form must contain all the details about the materials and services, including their cost.

What I need the Prop-003 for?

Contractors require this form to specify the information on:

• Costs of materials and employees payments;
• Guarantees that can be given about the materials and quality of work;
• Deadlines that were discussed between the contractor and the project owner;
• Extra costs if they are needed;
• Project owners receive the confirmation of the work that must be done.

The form does not have an exact due date. It is valid as long as the contract is active. Once all the agreed services are done and payments made, contractor and project owner sign final certificate, and the form Prop-003 is no longer significant.

Filling out Prop-003

To fill out Prop-003, you can be either a contractor or a project owner. In both cases, you will need the official sign of another part. Yet, it is easy to fill out once you own the following information:

• Personal data, including name, address, position;
• Location of the job;
• Sums that you need to pay for materials;
• Payments for the labor you can give to your employees.

After the Prop-003 form is signed by both contractor and project owner, it can also be notified by the third legal party. Once every line is filled out, you can send it to both the contractor and the project owner. It will be valid until all the conditions agreed in the document are fulfilled.
