What Is the SECNAV Form 5512-1 Used For?

The Department of the Navy (DON) requires that all employees (military, civilian, and contractors) wishing to gain unescorted access to DON facilities have a valid Department of Defense (DoD) identification card. How to Fill Out SECNAV Form 5512-1?
