PS Form 8125

What is PS Form 8125?

PS Form 8125, "Plant-Verified Drop Shipment (PVDS) Verification and Clearance", is a form of the United States Postal Service. Its instructions state its purpose: to prove to the entry facility that the mail being presented by the mailer or the mailer's agent was verified and paid for at origin. Plant-verified drop shipment lets a mailer have a shipment verified and the postage paid at origin and then transport it to destination Post Offices. The form refers to Domestic Mail Manual (DMM) 705.17 for the standards.

What is on the form?

The form has three sections:

• Mailer Information: the mailer's name and contacts, the origin plant location, the class of mail, the product or publication title, the total gross weight of the shipment, the pallets and other containers, and the entry discounts claimed.
• Origin Post Office (Where verified): where the mailing was verified, the permit number, the postage payment method (except for Periodicals) and the total pieces.
• Destination Entry Post Office or Delivery Unit: the facility where the mailing will be entered and its receipt there.

At the top are boxes for the requested in-home delivery date and the drop ship appointment number. The instructions say the appointment number is required and may be added by the mailer or the mailer's agent after the origin Post Office has signed and dated the form but before the mailing is presented at the destination.

Who completes which items?

According to the instructions on the form, the mailer must complete all items in the Mailer Information section, except for the pallet presort items it marks as optional, and also item 28, the facility where the PVDS mailing will be entered. The origin Post Office checks that the mailer has completed the required items, completes the Origin Post Office section, and signs and round stamps the form. At the destination, the instructions tell the entry office to accept the shipment if the form is properly completed and the information on it matches the mail, and to complete items 29 through 33; they also say what to do when the mail is visibly damaged or the shipment does not match the form.

How is the form submitted?

According to the form, the mailer submits an original and at least one copy of PS Form 8125, or an approved facsimile, with each PVDS mailing presented for verification and postage payment at origin. The original, with the original signature and round stamp of the origin Post Office, then goes with the mailing to the entry facility. USPS words the copies for the entry facility in two ways: the form says to submit a second copy if the mailer wants one signed by the entry office and returned for its records, while the Domestic Mail Manual (section 705.17.2.9) says the form must be submitted in duplicate, or in triplicate if the mailer wants a signed and dated copy returned. The two instructions differ on the number of copies. The form says it is not required for PVDS mailings sent via Priority Mail Express or Priority Mail Open and Distribute.
