Section 1 - Property Information:

You must attach a qualified appraisal if the claimed deduction for an item or group of similar items exceeds $5,000. A qualified appraiser should perform the appraisal and include relevant details about the appraised property.
Section 2: Interests and Restrictions
Choose the letter from Section B, Part I, which represents the partial property you donated. If you have multiple properties under Section B, Part II, please attach an additional note.
For the property in Section B, Part I, state the total deduction amount you're claiming:
- For this tax year.
- From previous tax years.
List their names and addresses if you've donated to different organizations in past years. Fill this out only if the recipient differs from the organization in Section B, Part V. Tell us where they're stored or located for physical properties.
Mention anyone besides the recipient organization who is holding onto the property.
State if there are any short-term or long-term limits on the recipient's ability to use or sell the donated property.
Did you grant rights to the property's income or possession to someone other than the receiving organization or another fundraising partner? If yes, this means privileges such as voting rights on donated stocks, buying the property, or selecting who benefits from the property's income or possession.
Indicate if there are any conditions on how the donated property should be used.
Sections 3 and 4 - Signatures
Donors and Appraisers should read the declarations in these sections and sign the form by clicking on the sign field. Once they signed the form, they must choose a date using the date field.
Section 5 - Acknowledgment:
This section requires the acknowledgment of the receiving organization or their representative. The acknowledgment verifies that the organization received the donated property and did not provide any goods or services in exchange, except for those mentioned.
