How to Fill Out the Massachusetts Lead Paint Disclosure Form

- Start completing the form by filling in the 'Landlord or Authorized Agent' section. Enter your full name, address, and phone number in the designated fields.
- Proceed to the 'Tenant(s)' section and provide the full name(s) of the tenant(s), their address, and phone number(s). Be sure to fill out the information for all tenants involved in the rental agreement.
- In the 'Property' section, type the complete address, including the street, city, state, and zip code for the rental property.

- In the 'Lead Law Information' section, click on the appropriate checkboxes to indicate whether you have provided the tenant(s) with the 'Tenant Lead Law Notification' and the 'Massachusetts Tenant Lead Law Notification'.
- Now, proceed to the 'Lead Inspection Information' section. If the rental property has undergone lead inspection, click the checkbox beside 'Lead Inspection' and provide the inspection date.
- In the 'Certification of Deleading Compliance' subsection, indicate whether a Letter of Full Compliance or a Letter of Interim Control has been issued for the property. Type the issuance date and the letter's expiration date in the provided fields if applicable.
- In the 'Tenant Acknowledgment' section, the tenant(s) must sign their name(s) and provide the date, confirming they received the required notifications and documents related to the Lead Law. They can create a digital signature on the PDFliner toolbar.
