  2. In the 'Company Name' text box, insert the name of your affiliated company as an influencer.
  3. Next two text boxes are for inserting your phone number and email address. Make sure to enter your active contact information.
  4. Scroll down to section labeled 'Invoice Details.' In the 'Invoice No.' field, type in the number corresponding to this specific invoice.
  5. You'll need to insert the date in the 'Invoice Date' field. Click on the calendar icon on the right side of the text box and select the appropriate date. If you're completing this on the day the invoice is meant to be sent, select today's date.
  6. Move to the section 'Service List'. In the 'Description' field, provide brief information about the service you provided as an influencer.
  7. Then move on to the 'Amount' field just to the right of the 'Description.' Here, you need to specify the amount you are charging for your services.
  8. The 'Subtotal' field underneath should automatically calculate based on the 'Amount' you entered if it doesn't, sum up the amounts of all services you listed and enter this total.
  9. In the 'Tax' field, input the taxation amount for your services. This should typically be a percentage of your subtotal, so calculate accordingly.
  10. The 'Total' field should sum up the 'Subtotal' and 'Tax' amounts; if it doesn't, add both amounts and enter the result here.
  11. Underneath 'Total,' you will see a drop-down option for 'Payment Method.' Click on this field and select your preferred payment method from the list.
  12. Finally, the 'Authorized Signature' section provides a field to add your signature. Click on the 'Signature' icon and choose 'Add Signature.' Draw your signature using your cursor or upload a photo from your device.
  13. The 'Date Signed' field is automatically filled out with date you complete this form. If you wish to add a different date, click the calendar icon and select an appropriate desired date.
