What Is IMM 5707 Family Information Form?

IMM 5707, Family Information Form (Visitors, Students and Workers), is the Immigration, Refugees and Citizenship Canada (IRCC) form on which an applicant lists their family members. Each person 18 years of age or older applying from outside Canada for a temporary resident visa, study permit or work permit must complete the Family Information form named in their application package: IMM 5707 or IMM 5645. IRCC's IMM 5707 instructions also require a minor under 18 who is travelling alone to complete the form. Whether you complete IMM 5707 or IMM 5645 depends on the application package for your country and on your document checklist, not on citizenship alone.

What I need the IMM form 5707 for?

For online applications, the portal's personalized checklist tells you whether to upload a family information form, and IMM 5707 is one of the two forms IRCC uses for it. It gives IRCC the details of your spouse or partner, parents and children, including who will accompany you to Canada.

Filling Out IMM 5707 Form

The IMM 5707 form has three sections.

Section A asks about:

• You: family name and given names as shown on your passport, name in your native language, date and country of birth, marital status, present occupation;
• Your spouse, common-law partner or conjugal partner: the same details, present address, and whether they will accompany you to Canada;
• Parent 1 and Parent 2 (mother or father): names, date and country of birth, present address, marital status, occupation, and whether each will accompany you to Canada.

Section B lists all your children regardless of age, residence or Canadian status, including married and adopted children, step-children, children adopted by others and children in the custody of a former partner or guardian; if you have none, check the box "I do not have any children", read Note 2 and sign its declaration (the same applies to Note 1 if you have no spouse or partner).

Section C is the declaration: sign and date it; an unsigned or undated form is returned.

After completing all the sections, select the format in which to download IMM 5707.

How to Sign IMM 5707 Form

IRCC asks you to fill its PDF in Adobe Acrobat Reader, press Validate, then sign and date the declaration in Section C; for online applications you upload the PDF as your checklist says and sign the application itself in your IRCC account by typing your name, and no long-form date or black-ink signature is required. If you apply on paper, sign the printed form in Section C.

What is the difference between IMM 5645 and IMM 5707

Both are IRCC Family Information forms for visitors, students and workers; the application package for your country lists which one to use. The two forms collect the same core family details (IMM 5645 also asks about your brothers and sisters); neither is a sponsorship form (sponsoring a spouse, child, parent or grandparent uses a separate application package).
