  8. Enter the amount paid or credited by installment due date in Columns (a) through (d).
  9. Enter the overpayment of the previous installment (see instructions).
  10. Calculate the total of Lines 4 and 5.
  11. Calculate the underpayment (Line 3 minus Line 6).
  12. Calculate the overpayment (Line 6 minus Line 3).
