DA Form 5960

DA Form 5960 is the Army's Basic Allowance for Housing (BAH) Authorization and Dependency Declaration. It is used to start, adjust or terminate a Soldier's entitlement to BAH and to verify the Soldier's dependency status. For BAH recertification, the Soldier signs the form and the company level commander certifies it; commanders may delegate the certification, in writing, to the first commissioned officer in the Soldier's chain of command.
