Certified Payroll form (WH347) has to be submitted to the government agency weekly. The form is a required document for all contractors and subcontractors that perform work on the contracts financed by federal funds or assisted construction contracts.

Filling out Certified Payroll form (WH347)

The form consists of one single page with a table to fill in. First of all, you need to enter the name of the contractor or subcontractor, their address, payroll number, and general information about the project. The table has nine columns:

  • Name and ID;
  • Withholding exemptions quantity;
  • Category of work;
  • OT/ST.;
  • Number of working hours for each day;
  • Total number of working hours;
  • Rate of pay;
  • Total earned amount excluding taxes;
  • Deductions (FICA, Withholding tax, Other, Total Deductions);
  • Weekly paid net wages. 
  • Remember that the form must be submitted every week.
