What is form 40V?

Form 40V is a tax form used for paying a tax liability. It is a universal form used through the country, however each state has its own version and this particular form is issued by the Alabama Department of Revenue. (In accordance with the IRS).

What do I need the form 40V for?

The 40V form can be used for tax paying by employed workers, business owners, and so forth. It includes: 

  • Amended tax return,
  • Automatic extension,
  • Tax liability for the individual tax return.

No other tax payments are possible with this document. Besides, this version is valid only within Alabama’s territory.

How to fill out form 40V?

To make the necessary payment, you need to use the voucher attached to the document below. Fill it out and mail it to the Alabama Department of Revenue with the payment itself. 
Form filling requires:

  • Personal info. Write your and your spouse’s names, telephone number, locations and ZIP code.
  • SSN. Enter yours and your spouse’s SSN.
  • Tax. Check the box next to what you’re paying for: automatic extensions payment, return or amended.
  • Payment. Write the money amount you are paying.

Note: All payment methods are valid, except for sending cash. You can use a credit card, money order, E-check, and so on.
