- Start with the 'Tax Year Beginning' field, and input the first day of the tax year you are filing for in the format MM/DD/YYYY. You should also enter the last day of this tax year in the 'Tax Year Ending' field.
- Fill in your 'Account Number' in the provided field, which is a unique number assigned to your business for tax purposes.
- The 'FEIN' field is where you include your Federal Employer Identification Number. This number is used by the IRS to identify your business on tax returns.
- In the 'Legal Name' field, write your legal business name as it appears on your tax returns.
- Populate the 'Mailing Address', 'City', 'State', and 'ZIP Code' fields with your official business address.
