How to Fill Out the 2015 Form W-3C
The process of completing Form W-3C might seem daunting, but with careful attention, it can be straightforward:
- At the top of the form, enter the employer's identification number (EIN), followed by the legal name and trade name of the employer. Ensure that these match the original W-3 form that is being corrected.
- Fill out the employer's address, including the street address, city, state, ZIP code, and email address.
- Indicate if your address has changed since filing the original W-3 by checking the appropriate box.
- Specify the kind of payer by checking the correct box, whether it's for Social Security wages, Medicare wages and tips, or both.
- Specify the kind of employer by checking the corresponding box – for instance, if you're a federal, state, or local government employer or if you're exempt from Medicare taxes.
- Check the box that applies to the establishment number or the total number of Forms W-2 attached to the form.
- Enter the total number of W-2 forms you're correcting with this W-3C form.
- Correct any amounts from the W-3 that is being rectified, entering updated numbers for wages, tips, and other compensation.
- Update federal income tax withheld along with Social Security wages, Social Security tips, and allocated tips as necessary.
- Correct any figures related to Medicare wages and tips, Social Security tips, and any advanced EIC payment entries.
- If there are amounts regarding dependent care benefits or nonqualified plans, update these values accordingly.
- Review all figures for accuracy, comparing them to the corresponding corrected W-2 forms to ensure everything matches.
