What is DD Form 2962?

DD Form 2962 is the Personnel Security System Access Request (PSSAR). Under its title the form names the Defense Counterintelligence and Security Agency (DCSA). Its Privacy Act statement gives the purpose: "To request the establishment of user roles and access and validate the trustworthiness of individuals seeking access to Defense Central Index of Investigations (DCII), DoW Secure Web Fingerprint Transmission (SWFT), DoW Defense Information system for Security (DISS) or National Background Investigation Services (NBIS)."

What are the parts of the form?

The form has five pages: four pages of the request and one page of instructions. It is divided into seven parts.

• Part 1, Personal Information (Items 1 to 13): the applicant's name, organization, office or department, contact details, job title and grade or rank, citizenship, date and place of birth, Social Security number and, for a contractor, the CAGE code. Item 13 is the designation of the applicant: Military, DoW Civilian, Industry or Non-DoW.
• Part 2, Applications (Items 14 to 19): six blocks, each with the type of request and the roles or permissions requested.
• Part 3, Training (Items 20 and 21): Cyber Awareness Training and Personally Identifiable Information Training, each with a date.
• Part 4, Applicant's Certification (Items 22 and 23).
• Part 5, Nominating Official's Certification (Items 24 to 29).
• Part 6, Validating Official's Verification (Items 30 to 38). The instructions say of this part: "Do not complete if self-nominating/validating."
• Part 7, the instructions for the items of the form.

Which systems does Part 2 cover?

Part 2 has six blocks:

• Item 14, Defense Central Index of Investigations (DCII), which the form marks "(GOVERNMENT ONLY)";
• Item 15, Secure Web Fingerprint Transmission (SWFT);
• Item 16, Defense Information System for Security, Joint Verification System (DISS-JVS);
• Item 17, Defense Information System for Security, Case Adjudication Tracking System (DISS-CATS);
• Item 18, Defense Information System for Security, Appeals;
• Item 19, National Background Investigation Services (NBIS).

Each block begins with the type of request. The instructions say: "Select "initial" for a new account, "modification" for a change in privileges to an existing account, "deactivate" to remove all access and disable an existing account. Enter User ID if selecting "modification" or "deactivate.""

Where does the completed form go?

The form says: "Return completed form to the appropriate Account Manager or DCSA Contact Center, as indicated in the instructions." The address printed above that sentence is for comments on the reporting burden, and the form adds: "PLEASE DO NOT RETURN YOUR COMPLETED FORM TO THE ABOVE ORGANIZATION."

The instructions point to "the respective Account Management Policy available from the DCSA website for supplemental guidance on completing and submitting this form".
