What is DD Form 1801?

DD Form 1801 is the DoD International Flight Plan, a form of the United States Department of Defense. The blank on this page is the edition marked "DD FORM 1801, MAY 87". The form prints "PREVIOUS EDITION IS OBSOLETE."

What is on the form?

The upper part of the form is the flight plan message:

• "PRIORITY" and "ADDRESSEE(S)", "FILING TIME" and "ORIGINATOR", and a line headed "SPECIFIC IDENTIFICATION OF ADDRESSEE(S) AND/OR ORIGINATOR";
• item 3, message type, where the form prints "(FPL";
• item 7, aircraft identification;
• item 8, flight rules, with the type of flight;
• item 9, number, with the type of aircraft and the wake turbulence category;
• item 10, equipment;
• item 13, departure aerodrome, with the time;
• item 15, cruising speed, with the level and the route;
• item 16, destination aerodrome, with the total EET in hours and minutes, an alternate aerodrome and a second alternate aerodrome;
• item 18, other information.

Below a line that reads "NOT FOR TRANSMISSION" comes item 19, supplementary information. It has the headings "ENDURANCE", "PERSONS ON BOARD" and "EMERGENCY AND SURVIVAL EQUIPMENT", with entries for the type of equipment, life jackets, the radio frequency and dinghies, and a place for other equipment. Its printed entries include "FUEL/", "POB/" and "RDO/" with "121.5", "243", "500" and "8364"; "POLAR", "DESERT", "MARITIME", "JUNGLE" and "GLOBAL" under "TYPE OF EQUIPMENT"; "JACKETS", "LIGHT" and "FLUORESCEIN" under "LIFE JACKETS"; and, for dinghies, "COLOR", "NUMBER", "TOTAL CAPACITY" and "COVER".

The last block has remarks, the aircraft serial numbers and type of aircraft in flight, a crew list and a passenger manifest (each with the choices "ATTACHED" and "LOCATED AT:"), the name of the pilot in command, the signature of the approving authority, and the aircraft home station or organization.
