  5. Write down the "Requestor Name". This could be your name or someone who is making the request on your behalf.
  6. Input the "Requestor Title", the official title of the person making the request.
  7. Now, move to the "Current Details" section. 
  8. Fill in your current "Division", "Department", "Job Code", and "Payroll Title".
  9. Type your "Working Title", the title you currently hold.
  10. State your Full-Time Equivalent (FTE) percentage in the field marked "FTE %".
  11. Enter your current "Union", "Personnel Program", and "FLSA Status" in their respective fields.
  12. Add in further details like your "Grade/Step", "Annual Base Salary", and "Salary Range".
  13. Mention how long you have been funded under "Funding Duration", and also include your "Fund #".
  14. Mark "Externally Funded" as Yes or No, depending on your circumstances.
  15. Provide the name of your "Supervisor" and "Dean, Vice Chancellor or Associate Chancellor".
  16. Proceed to the "Proposed Details" section, filling it out similar to the "Current Details", this time focusing on proposed changes.
  17. In the section about "Internal Promotional Increase/New Hire Salary", specify whether it's an "Existing or New Position" and select whether it's an "Internal Promotion or External New Hire", also providing either 25th % tile of salary range or previous incumbent's salary accordingly.
  18. Fill out the "Change Summary" section, providing the "Annual increase amount" and "Percent increase". Also, add the "Effective Begin Date" for these changes.
  19. In the "Department Justification" section, provide reasons and/or facts supporting your salary increase request.
  20. The last few sections are to be filled out by different approving authorities: "Compensation's Input", "Division Approval", and "Position Management Review Committee (PMRC) Approval". 
  21. Submit the form once you have ensured all entries are correctly filled.
