Report of Vision Examination (DL 62)
What is DL 62?
DL 62 is the Report of Vision Examination from the California DMV (California Department of Motor Vehicles).
If you fail the vision screening, you are referred to a vision specialist. DL 62 is the report the specialist completes.

Completing and submitting it
Enter your driver license number, birth date, telephone number, name and address, then sign and date the authorization line.
The specialist signature section must be completed to validate the report.
If or when you return to DMV for further testing, bring the completed form and any new corrective lenses.
Do not mail it back unless a DMV employee asks; see Vision Conditions guidance.
