What is OFW Information Sheet Form?

OFW Information Sheet Form or PH FM-POEA O2-GP-07 2005-2020 is a fillable federal form designed by the government of the Republic of the Philippines. It gathers personal information and contract particulars of OFW. The form is not followed by any other forms but requires a valid passport number and an active place of employment.

What I need the OFW Information Sheet Form for?

OFW Information Sheet Form:

  • Can be requested by Philippine Overseas Employment Administration, Overseas Workers Welfare Administration, and Philippine Health Insurance Corporation. 
  • Allows an applicant to provide personal information to a government institution for further processing.

The deadline for the submission of the form is not established. However, you are supposed to submit it as soon as possible after receiving an official request from one of the institutions listed above.

Filling out the OFW Information Sheet Form

To fill out the OFW Information Sheet Form, you have to provide your personal data, including Family name, First Name, Middle Name, Address in the Phils, Birth Date, Gender, Civil Status (marital status) and the name of the spouse, Mother’s full maiden name, the number of the valid passport of the Philippines, Highest Education Attainment, and other legal beneficiaries. The second section requires contract particulars of OFW. In this section, you have to provide the name of your Employer, Company, or Principal, the address of the place of employment, and other data. The form must be signed by the applicant (worker), and an agency representative.
