The form consists of parts for identifying the fiduciary, reporting income, deductions, and tax computation. The sections must be accurately completed to ensure a proper return and to avoid potential penalties for inaccuracy.

  1. Access the first section, where you’re expected to enter the Name of Funeral Trust. Make sure to note that abbreviations and special characters, other than hyphens and apostrophes, aren’t accepted.
  2. Proceed to the section termed "Employer Identification Number (EIN)", and add the appropriate 9-digit EIN as provided by the Internal Revenue Service (IRS).
  3. Now, navigate to the "Name and Title of Trustee" section, and enter the full name and legal title of the appointed trustee.
  4. Move to the "Number of Funeral Trusts" section, and provide the correct number of trusts for which the form is being filled.
  5. Continue to the “Income” section. For specific lines, indicate the specific incomes earned over the tax year by the trust.
  6. On the next lines, jot down the total income and distribute any applicable deductions respectively.
  7. Now, focus on the “Tax and Payments” section. On these lines, you will calculate the taxable income, tax on the income, and tax due after credits.
