What is form I-693?

To protect the health of the United States citizens, non-immigrant and immigrant foreigners must first prove that they are not carriers of dangerous diseases. Therefore, USCIS requires the visitors to submit Form I-693, in which the results of medical examinations are provided. These exams are needed to prove whether the applicant is eligible to enter the United States or not.

What I need the I-693 for?

Form I-693 is used as evidence that the applicant is not bringing anything into the country that could threaten the population. However, this medical examination should be provided by one of the doctors:

  • If you apply from the US: Contact a civilian surgeon who is a doctor appointed by USCIS;
  • Applying a statement from Abroad: Find a doctor who is an authorized U.S. Department of Health.

Filling out the I-693 form

In total, I-693 consists of 10 parts. The form is filled by the applicant, the doctor, and the interpreter (if needed). To fill the applicant part, you have to provide:

  • First, last, and middle names;
  • Full address;
  • Gender and the date of birth;
  • Country and city of birth;
  • A-number (alien registration number) if needed;
  • The USCIS online account number (if you have one).
