Who is this template for?
A medical practice, clinic or school health office that confirms, from its own records, that a person attended a visit. The person who signs it writes their own name and role in the "Issued by" part.
What does it contain?
The office name, address and contact; the name of the patient or student; an optional recipient organization and class or employee identifier; the date of the visit and, if needed, the arrival and departure times; an attendance statement; the date of issue; and the name, role, signature and signature date of the person who issues it. It has no field for a diagnosis. The form itself says: "This confirmation does not assess fitness or recommend absence. Give the person a copy."
