Step 6: The parent or legal guardian should read the authorization statement, provide their name, sign, and date, certifying that they give permission for the minor to obtain an ID card and that all information provided is accurate.

Step 7: If the minor wishes to become an organ donor, they can indicate their choice at the top of the first page. And their parent or guardian should give their consent by marking the appropriate box on the “Certification” block.

For males under the age of 26, a parent or guardian would also need to fill out the “Selective Service” block and sign it as well.
