California DMV Form SR 1 is a standard two-page report of traffic accidents that have occurred in California. Any participant (or their official representatives) in the incident must complete this document and submit it to the DMV within ten days of the date of the event.

• You must fill out a document if you are involved in a car accident.
• Complete DMV Accident Form SR 1 if a car crash has caused injury or death to people or damage to someone’s property over $1000.
• Do not send this document if no one has been hurt and the property is not badly damaged (for example, only scratches and dents are visible on the surface).

How to Fill Out the DMV SR-1 Form online?

1. Any party to an incident can complete this form. So first, you need to complete the block regarding one participant and then the other (regardless of fault).
2. Fill in the parties’ full names, addresses, vehicles, license plate numbers, insurance company details, insurance expiration dates, etc.
3. List the number of vehicles involved in the accident, the time and place, and where each participant was at the time.
4. Complete the third block, which is dedicated to injured or dead people and damaged property.
5. The filler must enter their name at the end and sign. If you need more space to describe the accident, attach a piece of paper to the document.
