Completing and submitting it

Use black ink and CAPITAL LETTERS. Sign and date your application.

For another vehicle's registered keeper details, apply by post using DVLA's instructions.

Those instructions name V888 as the form for an individual and other forms for companies. A company that issues parking or trespass charge notices uses V888/3.

DVLA says private car parking management companies that give out parking tickets or trespass charge notices can only request information from DVLA if they are members of the British Parking Association or the International Parking Community.

A company that does not issue parking or trespass notices and wants information about a vehicle on a specific date uses V888/2A to request the keeper's name and address, or V888/2B to request any other information about the vehicle.

DVLA's subject access request form covers information held about you, your current vehicle or a vehicle previously registered to you.

Check DVLA's instructions before you submit the form.
