Filling out Empty Container Release Form

Empty Container Release Form consists of 3 fillable sections. Each section should include a printed name, the date of the operation, and the signature of the person responsible for the operation. The 1st section should additionally include the address of the initial location of containers on the WTAMU campus. The 3rd section should include the end disposal location. Besides, sections 1 and 2 should include locations of the personnel responsible for rendering and proper disposal of containers. Return the filled form to the West Texas A&M University. Copies of the filled form can also be kept by the above-mentioned personnel if necessary.
