What is the Empty Container Release Form?

Empty Container Release Form is designed to collect personal information of an applicant who receives containers from a specific location and names of the personnel who releases containers. The Empty Container Release Form isn’t accompanied by other documents but must include valid signatures of the responsible personnel.

What I need the Empty Container Release Form for?

Empty Container Release Form:

• Is required to verify that you have released unusable containers from the location that you are responsible for;
• Is needed to confirm that you have received a container(s) and undertake to take them to the proper disposal facility. 
• Has to be signed by the third-party facility that receives containers for disposal.

The form has to be filled and signed by all participants of the transportation and disposal process at the moment when they commit the agreed task. Other time limitations are not envisaged by WTAMU.

Filling out Empty Container Release Form

Empty Container Release Form consists of 3 fillable sections. Each section should include a printed name, the date of the operation, and the signature of the person responsible for the operation. The 1st section should additionally include the address of the initial location of containers on the WTAMU campus. The 3rd section should include the end disposal location. Besides, sections 1 and 2 should include locations of the personnel responsible for rendering and proper disposal of containers. Return the filled form to the West Texas A&M University. Copies of the filled form can also be kept by the above-mentioned personnel if necessary.
