What is the Employee Performance Review form?
Employee Performance Review is the form that is used by team leaders, reviewers or managers as a way to express their thoughts on employee performance (for example, employee of the month). It may contain important questions about work duties, skills, initiative, and a number of days-off. There is a 4-grade evaluation used in this form: poor, fair, good, and excellent.
What I need the Employee Performance Review for?
This form is required to be filled in case:
- Manager, leader of the team or reviewer wants to evaluate the skills of the employee;
- Employee wants to receive a promotion or a bigger salary and asks for this review;
- The company requires to see the evaluation of employee’s productivity at work, quality, skills, attendance, place in the team.
Employee Performance Review does not have a due date. This form can be completed at any time when the employer made a request. Usually, big companies fill out Employee Performance Review forms once in 6 or 3 months.
Filling out Employee Performance Review
The form is filled by the manager or another reviewer. It must be shown to the employee. The Employee
Performance Review form has to contain the following information:
- Name of the employee;
- Department where the employee works;
- Reviewer personal information: full name and position;
- The exact date when the review has been made;
- Period of time during which the review took place;
- Title of the reviewer;
- Skills and their type;
- Evaluation of performance: poor, fair, good or excellent;
- Suggestions for the further development of employee;
- Comments from the reviewer.
Both the reviewer and the employee have to sign this form before applying it. The reviewer may add additional information about the employee in this form. Employee Performance Review form will be attached to the personal file of the employee. This form will be kept in the HR department.
