What is Form V1?
Form V1 is the DVLA (Driver and Vehicle Licensing Agency) "Vision self declaration" for car drivers or motorcyclists reporting certain visual conditions.
DVLA recommends reporting online. You can also use V1 if you cannot use the online service.
If you hold a lorry, bus or coach licence, use a different form for these visual conditions.
Edition shown
The questionnaire is marked "Rev Sept 25"; its Applicant's Authorisation is marked "Rev July 22".
DVLA publishes V1 on its "Report your medical condition (form V1)" page.
Completing and submitting it
Answer the questions in block capital letters using black ink.
Read the Applicant's Authorisation information carefully and sign to confirm its statements.
Return the form to the Drivers Medical Group by post or email.
