Huntington Bank Direct Deposit Enrollment Form

What is the Huntington direct deposit form?

The form is headed "DIRECT DEPOSIT ENROLLMENT FORM" and carries the Huntington name. With it an employee authorizes the payroll department of an employer "to establish Direct Deposit of my paycheck into the account listed below". The form prints no form number and no edition date.

Huntington's page on setting up a direct deposit lists the steps: fill out a Direct Deposit Form, identify your routing and account number, provide a blank, voided check if your employer requests one, and submit the form.

What is on the form?

The form has three numbered steps.

1. "Complete this form by filling in all the requested information below." The fields are First Name, Last Name, Address, City, State and ZIP Code. Below them is the sentence that authorizes the payroll department, with a blank for the name of the employer. A note says: "This form will contain confidential banking information and should be kept secure."
2. "If your employer requires a voided check, please staple here. If not, please include your account information in the space provided below." A sample check marks the place for the voided check and shows where the "Bank Routing Number" and the "Account Number" are printed on a check; beside it are the fields Banking Routing Number and Account Number.
3. "Drop off this completed form at your employer's HR department."

The form has no line for the employee's signature and no line for the date of signing.

Where are the routing and account numbers found?

Huntington's page says: "Identify your routing and account number, which you can find at the bottom of your checks. You can also find those numbers online (on the account summary tab) when you are logged into Huntington Online Banking or by visiting a Huntington branch."

Where does the form go?

The completed form goes to the employer: the form says to drop it off at the employer's HR department, and Huntington's page says, "Once the form is filled out, simply give it to your employer's HR department."
