What is D2?
Form D2 is the "Application for a lorry, bus or minibus driving licence" of the DVLA (Driver and Vehicle Licensing Agency).
GOV.UK's guidance for a provisional HGV or bus licence says to fill in two forms: D2 and a D4 medical examination report, which must be filled in by a doctor.
How DVLA provides this form
D2 application packs are only available from a Post Office that offers DVLA services.
The DVLA forms service cannot be used to order D2.
We found no D2 download on the DVLA forms page or its linked download service when we checked on 4 October 2026.
GOV.UK provides guidance for applying for a provisional HGV or bus licence.
