What is Aflac form S13270?

S13270 is the Aflac (American Family Life Assurance Company of Columbus) Continuing Disability Claim Form. This page does not offer a document or an editor. Aflac says claim forms are state specific: choose the policyholder's state of residence on Aflac's fax or mail claim page and select the appropriate form.

Who completes which part

  • Policyholder or patient: policyholder information (fields marked * are required), then signature and date.
  • Employer: the Employer's Statement, including whether the employee has returned to work, the expected or actual return date, and weekly hours if working part-time or light duty.
  • Physician: the Physician's Statement, including the primary diagnosis and ICD code, the next appointment and expected release date, signature and tax ID number.

The form says that failure to complete all sections may delay processing, asks for documentation from your healthcare provider to support the claim, and says medical records will be required if permanent disability is indicated.

How to file

Aflac's fax or mail page says to complete your claim forms and send them with all necessary supporting documentation. You can also file online by registering on Aflac.com or through the MyAflac mobile app. Use the form for your state from Aflac's page; it lists the current fax number and mailing address.
