What is Form MV-16?
Form MV-16 is the Affidavit to Certify Immediate Family Relationship from the Georgia Department of Revenue, Motor Vehicle Division.
Georgia residents use it as evidence that a vehicle was transferred between immediate family members (spouse, parent, children, step-children, sibling, grandparent or grandchild), whether gifted, purchased or inherited.
