How To Fill Out Form IT-2104-SNY 

Step 1: Determine Your Eligibility 

Before filling out the form, confirm that you meet all the qualifying conditions. You must be employed by a START-UP NY-approved business, work exclusively in a tax-free NY area, be employed in a net new job, and work or expect to work at least six months in the tax year at the business's location.

Step 2: Fill Employee Information 

In the "Employee information" section, provide your details:

  • Enter your first name, middle initial, and last name.
  • Include your mailing address with the apartment number, if applicable.
  • Provide your city, state, and ZIP code.
  • Write down your Social Security number.
  • Mention your filing status by marking 'X' in the appropriate box representing your filing status, which could be single, married, filing a joint return, or head of household.
  • Fill in your date of birth in the format.

Step 3: Certify Your Information

Read the certification statement that confirms you are eligible and expect to qualify for the exemption. If you agree and meet the conditions, sign and date the form.

Step 4: Employer Information 

Although this section is for your employer to complete, ensure they include their business name, Employer Identification Number (EIN), and certificate number. You don't need to fill in this part as an employee, but your employer must do so for their records.

Step 5: File the Form

Once completed, give the form to your employer and retain a copy for your records. Do not send this form to the New York State Tax Department. Remember, you must file a new form, IT-2104-SNY, each year if you remain eligible and wish to claim the exemption.
